7 Basic emotions: overview of TCM



The seven basic emotions related to organ function are

Anger, 
Joy, 
Worry, 
Pensiveness, 
Sadness, 
Fear, 
and Shock (fright). 

Although the mind/body connection has been acknowledged only relatively recently in Western medicine, the interaction of emotions with the physical body is an essential aspect of traditional Chinese medicine.
Each organ has a corresponding emotion; imbalance of this emotion can affect the organ's function. For example, prolonged anger can lead to an imbalance in the liver. At the same time, liver imbalances can produce symptoms of anger, often leading to a self-perpetuating cycle.

In discussing the emotional aspect of the disease process, it is important to remember that it is normal to experience the full range of emotions. It is only when a particular emotion is experienced over a prolonged period or with particular intensity that it becomes a source of imbalance.

Anger 

Anger is associated with the liver. By its nature, anger causes qi to rise, leading to a red face and red eyes, headaches, and dizziness. This matches the pattern of liver fire rising. Anger can also cause liver qi to "attack the spleen," producing lack of appetite, indigestion, and diarrhea (often experienced by those people who argue at the dinner table or eat while driving).

In a more long-term view, suppressed anger or frustration often causes liver qi to become stagnant; this might result in depression or menstrual disorders. It is interesting to note that people who take herbs to release stagnant liver qi often experience bouts of anger as the stagnation is relieved. The anger passes as the condition clears. Similarly, anger and irritability are often the determining factor in diagnosing liver qi stagnation. Many people are relieved to know their rage has a physiologic basis. It is essential to avoid drinking coffee when treating anger-related liver disorders, as coffee heats the liver and greatly intensifies the condition.

Joy


The emotion of joy is connected with the heart. A disorder related to joy may sound perplexing, since most people want as much joy in their life as possible. The disorders from this emotion are not caused by happiness; rather, the imbalance comes from too much excitement or stimulation, or sudden good news that comes as a shock to the system.

When evaluating stress levels, psychologists look at all sources of stress, both positive and negative. Clearly the death of a spouse or a job loss is a significant source of stress. However, a marriage or job promotion, while a happy occasion, is also a source of stress. A person who is constantly on the go, partying, and living a life of excess can eventually develop heart imbalances with palpitations, anxiety, and insomnia. A person with heart imbalances may also exhibit emotional symptoms, since the heart is the seat of the spirit (shen). A person with extreme disturbances of heart shen might be seen chattering happily to himself with outbursts of laughter.

Such behavior results from the heart organ's inability to provide a stable resting place for the spirit. This type of imbalance is treated with acupuncture along the heart meridian. Herbal treatments consist of formulas that nourish heart blood or yin. If heart fire disturbs the spirit, herbs that clear heat from the heart are used.

Worry


A very common emotion in our stress-filled society, worry can deplete the energy of the spleen. This can cause digestive disturbances and eventually lead to chronic fatigue: A weakened spleen cannot efficiently turn food into qi, and the lungs are unable to extract qi from air efficiently. A person who worries too much "carries the weight of the world on her shoulders," a good description of how a person feels when her weak spleen qi leads to dampness. Treatment would include moxa and herbs that strengthen the spleen, allowing a person the energy to deal with life's problems instead of dwelling on them.

Pensiveness


Too much thinking or obsessing about a topic can also deplete the spleen, causing a stagnation of its qi. A person with this condition may exhibit such symptoms as poor appetite, forgetting to eat, and bloating after eating. In time, the person may develop a pale complexion from a deficiency of spleen qi. This can eventually affect the heart, causing the person to dream about the same subjects at night. Students are often affected by this imbalance; the standard treatment is use of herbs that tonify heart blood and spleen qi.

Sadness

Sadness or grief affects the lungs, producing fatigue, shortness of breath, crying, or depression. Treatment for this condition involves acupuncture to points along the lung and kidney meridians. Often, herbal formulas are used that tonify the qi or yin of the lungs.

Fear


The emotion of fear is related to the kidneys. This relationship can readily be seen when extreme fear causes a person to urinate uncontrollably. In children, this can also manifest as bed-wetting, which psychologists have linked to insecurity and anxiety. Long-term anxiety due to worrying about the future can deplete the kidneys of yin, yang, and qi, eventually leading to chronic weakness. Treatment involves tonifying the kidneys with yin or yang tonics, depending on the particular symptoms.

(Fright) Shock


Shock is especially debilitating to the kidneys and heart. The "fear or fright" reaction causes an excessive release of adrenaline from the adrenal glands that sit on top of the kidneys. This causes the heart to respond with palpitations, anxiety, and insomnia. Chronic stress from shock can be very debilitating to the entire system, causing a wide range of problems. Severe shock can have a long-term effect on the heart shen, as is evident in victims of post-traumatic stress syndrome. Treatment involves psychotherapy, herbs that calm the spirit and nourish the heart and kidneys, and regular acupuncture treatments.

Scleroderma: autoimmune skin disorder


Scleroderma
 is a chronic systemic autoimmune disease (primarily of the skin) characterized by fibrosis (or hardening), vascular alterations, and autoantibodies. There are two major forms:
Limited systemic sclerosis/scleroderma involves cutaneous manifestations that mainly affect the hands, arms and face. It was previously called CREST syndrome in reference to the following complications: 
Calcinosis, 
Raynaud's phenomenon, 
Esophageal dysfunction,
Sclerodactyly, and 
Telangiectasias. 
Additionally, pulmonary arterial hypertension may occur in up to one-third of patients, and is the most serious complication for this form of scleroderma.
Diffuse systemic sclerosis/scleroderma is rapidly progressing and affects a large area of the skin and one or more internal organs, frequently the kidneys, esophagus, heart and lungs. This form of scleroderma can be quite disabling. There are no treatments for scleroderma itself, but individual organ system complications are treated.

Causes:
The cause is unknown. Scleroderma runs in families, but the genes have not been identified. It affects the small blood vessels (arterioles) in all organs. First, the endothelial cells of the arteriole die off, along with smooth muscle cells, by a process of apoptosis. They are replaced by collagen and other fibrous material. Inflammatory cells, particularly CD4+ helper T cells, infiltrate the arteriole, and cause further damage. Many of the inflammatory and destructive protein signals have been identified, and they are potential targets for drugs that could interrupt the process.

Classification
Scleroderma is characterized by the appearance of circumscribed or diffuse, hard, smooth, ivory-colored areas that are immobile, and which give the appearance of hidebound skin, a disease occurring in both localized and systemic forms:
  • Localized scleroderma
    • Localized morphea
    • Morphea-lichen sclerosus et atrophicus overlap
    • Generalized morphea
    • Atrophoderma of Pasini and Pierini
    • Pansclerotic morphea
    • Morphea profunda
    • Linear scleroderma
    • Systemic scleroderma
    • CREST syndrome
    • Progressive systemic sclerosis

Diagnosis:
Typical scleroderma is classically defined as symmetrical skin thickening, with about 90% of cases also presenting with Raynaud's phenomenon, nail-fold capillary changes, and antinuclear antibodies. Patients may or may not experience systemic organ involvement. Atypical scleroderma may show any variation of these changes without skin changes or with finger swelling only. Additional symptoms of scleroderma typically present themselves within two years of Raynaud's phenomenon.

Lab Investigations:
Laboratory testing can show antitopoisomerase antibodies (causing a diffuse systemic form), or anticentromere antibodies (causing a limited systemic form, and the CREST syndrome). Other autoantibodies can be seen, such as anti-U3 or anti-RNA polymerase.

Severe complications from scleroderma includes:

Heart: Untreated high blood pressure strains the heart; irregular heart rhythm and enlargement of the heart lead to heart failure.

Kidneys: Scleroderma renal crisis in which malignant hypertension develops and causes acute renal failure was once a common cause of death, but is now treatable with ACE inhibitors.

Lungs: Two-thirds of all patients suffer from respiratory problems, such as shortness of breath, coughing, difficulty breathing, alveolitis(inflammation of lung air sacs), pneumonia, and cancer.

Digestive: Esophagus damage can make it difficult to swallow food, and acid reflux is common. The stomach can develop gastric antral vascular ectasia (GAVE), which occasionally may bleed profusely. A sluggish intestine may cause pain and bloating; undigested food can result in diarrhea, weight loss and anemia.

Skin and joints: Carpal tunnel syndrome is common, as are muscle weakness, joint pain, and stiffness.

Mouth: Flat white patches, loss of attached gingival mucosa, gingival recession, and diffuse widening of the periodontal ligament (PDL) space are seen. Dysphagia may result from collagen deposition in the lingual and esophageal submucosa. Resorption of posterior ramus of the mandible, coronoid process, and condyle are seen due to pressure from abnormal collagen production in adjacent areas. Inelasticity of the mouth may make dentures or dental prostheses difficult to insert and remove.

Murmur and Types of Valvular Heart Disease




Mitral valve prolapse: Normally our mitral valve closes completely when our left ventricle contracts, preventing blood from flowing back into our left atrium. If part of the valve balloons out so that the valve does not close properly, one have mitral valve prolapse. This causes a clicking sound as our heart beats. Often, this common condition is not serious. However, in rare cases it leads to infective endocarditis or mitral regurgitation (backward blood flow through the valve); both can be serious.

Mitral valve or aortic stenosis: Our mitral or aortic valves, both on the left side of our heart, can become narrowed by scarring from infections, such as rheumatic fever, or may be narrow at birth. Such narrowing or constriction is called stenosis. In mitral valve or aortic stenosis, the heart has to work harder to pump enough blood to satisfy your body's oxygen needs. If untreated, stenosis can wear out our heart and can lead to heart failure.

Aortic sclerosis: One in three elderly people have a heart murmur due to the scarring, thickening, or stiffening (sclerosis) of the aortic valve. This condition is generally not dangerous; typically, the valve can function for years after the murmur is detected. Aortic sclerosis is usually seen in people with atherosclerosis, or hardening of the arteries.

Mitral or aortic regurgitation: Regurgitation (backward flow) of blood can occur with mitral valve prolapse or mitral valve or aortic stenosis. To counteract this back flow, the heart must work harder to force blood through the damaged valve. Over time, this can weaken and/or enlarge the heart and can lead to heart failure.

Congenital heart defects: About 25,000 babies are born each year with heart defects, such as holes in heart walls or misshapen heart valves. Many congenital heart defects can be corrected by surgery.
Other Possible Causes of Heart Murmurs

Some common conditions can force your heart to beat faster, changing the rate and amount of blood moving through your heart and resulting in heart murmurs. You and your doctor should monitor all of these conditions:

· Pregnancy
· Anemia
· High blood pressure
· Overactive thyroid
· Fever

Parameters of a Murmur during examination:

§ Timing refers to whether the murmur is a systolic or diastolic murmur.

§ Shape refers to the intensity over time; murmurs can be crescendo, decrescendo or crescendo-decrescendo.

§ Location refers to where the heart murmur is usually auscultated best. There are six places on the anterior chest to listen for heart murmurs; each of the locations roughly corresponds to a specific part of the heart. The first five of the six locations are adjacent to the sternum. The six locations are:
                 the 2nd right intercostal space
                 the 2nd to 5th left intercostal spaces
                 the 5th left mid-clavicular intercostal space.

§ Radiation refers to where the sound of the murmur radiates. The general rule of thumb is that the sound radiates in the direction of the blood flow.

§ Intensity refers to the loudness of the murmur, and is graded on a scale from 0-6/6.

§ Pitch can be low, medium or high and is determined by whether it can be auscultated best with the bell or diaphragm of a stethoscope.

§ Quality refers to unusual characteristics of a murmur, such as blowing, harsh, rumbling or musical.


Grading of murmurs

Grade 1                   Very faint

Grade 2                    Soft

Grade 3                    Heard all over the precordium

Grade 4                    Loud, with palpable thrill (i.e. a tremor or vibration felt on palpation) 

Grade 5                    Very loud, with thrill. May be heard when stethoscope is partly off the chest.

Grade 6                    Very loud, with thrill. May be heard with stethoscope entirely off the chest.


Tests for Heart Murmurs

Usually heart murmurs are detected during a physical exam. Your doctor will be able to hear your heart murmur when listening to your heart with a stethoscope.

Your doctor may order one or more of the following tests to see whether your heart murmur is innocent or whether it is caused by acquired valve disease or a congenital defect:

· Electrocardiogram (ECG), which measures the electrical activity of the heart

· Chest X-rays to see if the heart is enlarged due to heart or valve disease

· Echocardiography, which uses sound waves to map the heart structure

Adrenalinum: Extracted from Materia Medica Viva By G Vithoulkas



 Adrenalin is also called Epinephrine
It is a sarcode and it is obtained as an extract from the adrenal or suprarenal glands. In Homeopathy we use the tincture or trituration. Practitioners should take special care when using this extract, to find out the exact composition origin of the remedy used by the pharmacy.

 Physiological action
Adrenalin or epinephrine is a 3, 4 - dihydroxy-a(methylaminomethyl) benzyl alcohol. It is a catecholamine and the chief neurohormone or neurotransmitter of the adrenal medulla. It has a sympathomimetic action on adrenergic alpha and beta receptors resulting in increased heart rate and contraction, vasoconstriction or vasodilation, relaxation of bronchiolar and intestinal smooth muscle while corresponding sphincters are stimulated, lipolysis, etc.

Epinephrine and generally catecholamines increase metabolic rate, and stimulate the breakdown of stored fuel with the production of substrate for local consumption; for example glycogenolysis in the heart. It also contributes to the regulation of the volume and composition of extracellular fluid, promoting cellular uptake of potassium in the renal tubule. Epinephrine along with the other catecholamines - norepinephrine and dopamine, also influence the secretion of renin, insulin, glucagon, calcitonin, parathormone and possibly testosterone.

In the viscera, especially the stomach and intestines and less so in the uterus and skin, Adrenalin causes constriction of the peripheral arterioles resulting in a rise in blood pressure. It also increases glandular activity, promotes glycosuria, depresses the respiratory center, and promotes contraction of muscular tissue of the eye, uterus and vagina while relaxing the muscular tissue of the stomach, intestines and bladder.
 In classical medicine it is used in the treatment of bronchial asthma [status asthmaticus], acute allergic disorders, open-angle glaucoma and heart block and as a local vasoconstrictor.

 The essential features
 This remedy - when properly proved and understood - will, I believe, become one of our major assets in combating contemporary diseases such as hypertension, diabetes, paroxysmal tachycardia with anxiety, arteriosclerosis and in general diseases that have come about because of the stresses of contemporary life. It is a remedy from which we could learn a lot, should we be able to test it in high potencies on animals or humans under stress, in order to ascertain how far it could counteract such stress.

 In the few cases where I have made use of Adrenalin, it was always in connection with underlying stress and high blood pressure. They were cases that looked very much like belladonna patients, with throbbing paroxysmal headaches, with hot body and cold extremities, with flushed face, tachycardia and generally a strong and full pulse, and on whom belladonna had an insignificant effect.

Since almost everybody today is under "a stress syndrome" I believe this remedy will be very frequently indicated, either to complete a cure or to open a case. I believe that Adrenalin will be a specific when the stress is of an emotional, psychological nature and is connected with continuous subconscious stress which has as a first indication the elevation of the blood pressure. Selye has also demonstrated through his theory of the "general adaptation syndrome" the direct relation between stress and production of adrenalin and noradrenalin.

 It is more than clear that the production of these two medulla hormones is directly connected with the defence system of the body. Their characteristic is that they are excreted in great quantities under stress and are retracted equally easily. Their amount in the bloodstream therefore varies widely under different circumstances.

 Most of my cases in which Adrenalin has shown some effect were in men with high blood pressure whose extra-marital affairs were of a complicated nature that caused them a lot of distress and inner unexpressed anxiety.

 The extreme explosion of the "idiopathic blood pressure syndrome" in our modern times seems to be related to a great extent to an abnormal production of Adrenalin caused by such stresses. From this logical sequence it will appear that the role of Adrenalin in treating this type of diseases with high potencies will be great in the near future.

 Adrenalin should be effective for instance in cases of paroxysmal tachycardia with high blood pressure. It will also be needed in cases of diabetis melitus where it is preceded by a history of hypertension and a life of constant and unresolved stress. Due to long term pressure on blood vessels and the increase of lipids in the blood the manifestation of arteriosclerosis, with cardiac distress, or even cardiac asthma, will also come under the action of the remedy.

 Other pathological conditions which we may think of for Adrenalin are: Pheochromocytoma, renal insufficiency, eclampsia, malignant hypertension with arteriosclerosis. It is indicated in cases of cardiac infarction or angina with severe constriction across the chest, anguish, vertigo, nausea and vomiting, hyperthyroidism. It is useful in congestion of the lungs, especially because of heart failure,
 and in cases where we may have sudden hemorrhages from all parts of the body, ie. nose, throat, larynx, stomach, rectum, uterus, bladder etc. Clarke says "it has cured a case of haematuria accompanied by pain in the adrenal region".

 There is progressive loss of strength and loss of weight and eventually emaciation. The pulse is very rapid and strong.
 In violent paroxysmal tachycardia with fear of dying and high blood pressure it is almost a specific. It causes constricting pains in the intestines with anxiety and tachycardia; metrorrhagia during and after labor in hypertensive women.

 Paroxysmal pulsating headaches with perspiration. Feeling of strong pulsation in the abdomen or in the whole body. Headaches during hypertensive crisis.

 The similarity of pheocromocytoma headaches and belladonna is striking. I quote from Harrison's Principles of Internal medicines: "Tumors that produce epinephrine predominantly are associated with prominent sweating, palpitations, nervousness, tremor, weight loss, pallor followed by flushing, heat intolerance, and rarely with hypotension.....Particular attention should be given to the three most common symptoms, headache, excessive sweating and palpitations, since at least one of these symptoms is almost always present. Headache is the most common. The headaches are abrupt in onset, severe, throbbing, generalised, and relatively short in duration..." We could not have a better description of belladonna headaches.

 Clinical
 Hypertension. Arteriosclerosis. Angina pectoris. Hemorrhages. Hyperemia.
 Hemophilia. Acute congestion of lungs. Asthma. Hay fever

 From 6x up to 200.

Requisites of a good Homoeopath...

While doing BHMS I always wondering that we are studying allied subjects like anatomy, physiology, surgery, medicine, pathology etc along with Homoeopathic subjects. It was an opportunity to be all rounder but when I sat in Doctor’s chair it didn’t work. I always wonder if I have gone through these entire subjects, why I am lacking in diagnosis or case taking or medicine selections.

I started my introspection and come to few conclusions. I am sharing these thoughts with you.



Homoeopathy is having a life triangle which we all know about. This is HMM, Organon and Repertory. For eg.: if we are considering Homoeopathy as a tool kit to fix a human, then organon is the users manual supplied for how to use best of Homoeopathy, while HMM is the bundle of fixer tools spans and screw drivers which can be used in different conditions for different nuts. Repertory is the technique or software which helps one to decide which group of span is most suitable for said situation.

But what about the different allied subjects which we learned during our academic years? Hahnemann has clearly defined about the requisites of homoeopathic physicians in organon apho 3 and 4. This says that a physician should first understand the body where the disease impact is seen during the process (because it is clear that the disease starts with derangement of the vital energy and then comes to the surface of body.) Hahnemann has stated that his best invention was the “Theory of Chronic disease” where Miasms are of utmost importance. Thus to understand the miasm of the disease pathology one should know about the normal physiology and anatomy.

For Materialistic study of Human:

Knowledge of Embryology
I was also wondering about the need of this subject when Dr P. Vijayakar said it. But yes, this is a fact that unless you are not aware of evolution of human body from one cell to one trillion cells sophisticated human body one cannot be confident of CURE. In olden era this knowledge was combined in anatomy and physiology. But we all know this is a complete separate subject. This helps in understanding the disease process and application of Hering Movement law during the curing process, which makes the subject of core importance.

Knowledge of Anatomy and physiology:  
This is the basic knowledge about the human body. It starts from the structure of cell till functions of each system. One must be through in all aspects of these subjects. After embryology these subjects makes one know about the whole body especially in healthy condition. Unless one doesn’t know about health one cannot treat a disease.
The knowledge of pathology will help in prognosis of disease as well as the clear picture of the miasm. This will also help during the follow up to guess whether the patient is getting cured or suppressed. Different lab test and diagnostic investigations will help to keep a close eye on the progress after giving a specific similimum.

Knowledge of medicine 
will fortify your miasmatic diagnosis and differential diagnosis. Also this can help in keeping the disease data for documentation of cases which one can present to show how Homoeopathy works.

Gynecology & obstetrics, Surgery, Pediatrics, Dermatology, ENT, ophthalmology and all the supportive systemic studies makes a Homoeopath fully prepared materialistically.

For further immaterial studies:

Knowledge of Miasms: 
 Dr. Hahnemann was very humble when he declared “Theory of Chronic disease” as his biggest discovery in Homoeopathy. So nobody can be a master unless, one knows about Miasms. We all know that there are only three miasm according to Hahnemann. Psora, Sycosis and Syphilis. All these three are like the defense mechanism of the cell. Cell is called the unit of life and thus the functions of cell includes the
Nutrition – Psora
Reproduction – Sycosis
Destruction or protection – Syphilis.
These are the reactions to the different needs of the body to survive. As survival of the fittest is the nature’s law.

Knowledge of emotions:  
This has a wide range of subjects like
Temperaments
Endocrinology and hormone effects on behavior
Subjective feelings; Objective emotions
Morals and Innate characters

Knowledge of Observations:  
This is quite important because this is an art part and a skill to be developed by the physician. This comes after a long practice and is not everybody’s cup of tea. What to observe and for what is the basic part of it. Observations are the best part as these confirms many thing during case taking.

IGNATIA: the other vision



IGNATIA AMARA
This is a well known remedy for hysteria and effects of grief. We are all aware of the sensitivity of Ignatia. But I want to share few more points which can be helpful while prescribing. Ign is a female dominant remedy which runs through out the picture.

Emotional Roller Coasteris the keyword used by Dr. Bailey for IGN. Thus contradictions are common for IGN. But I am trying to focus few more points which we face generally but are never discussed. 

Love and respect: 
This is the psora of Ign. Since childhood Ign girl tries to seek love and affection at home from parents or siblings and friends. Here need is love with respect and any one who knowingly or unknowingly minimizes this, hurts her. Ign is such a sensitive person that even parents who are involved in there jobs if not able to give attention can hurt her and bring a feeling of neglection.

Insecurity: 
Relationship is a rope walk for Ign. Initially don’t want to build a relation but if developed then fear of being hurt and neglected starts.

Reserved: 
This comes from a neglected feeling, as we all know that Ign is a sensitive person. Thus whenever she gets hurt she tries to get away from the persons who hurt her. This avoidance of the being hurt indirectly makes the person away from the group. She never tries to disclose the feeling but goes on sobbing on the same issues, thus making her a reserved personality.

Physical appearance:  
A sweet and sober looking women with aesthetic sense of fashion and exclusive style. She is beautiful and tries to be exclusive even in her acquaintances and relations. Physically slim and some time rigid with long delicate bones. The face tends to be angular rather than rounded, and the cheek bones are generally high, which is one reason why Ign are often sought after models. French women are good example as Ign says Dr Bailey.

Strong IGN:  
A counter part is a masculine Ign. This ignatia we have to deal with the female counterpart of Nux vom. By this I simply mean, that it suits the nervous temperament, the sensitive easily excitable nature that is quick in perceiving, prompt in appreciation and rapid in execution. This is very different from the thin irritable quarrelsome spiteful and the nervous disposition of Nux vomica, though masculine IGN has loving heart some where beneath. IGN is fond of social position and is very much cautious about it. Never want to be in the bad book like silicea.

Nearby remedies:
 
Nat- Mur: It resembles in all angles. But Ign is used for recent or on going grief, while NM is for chronic one. NM is hot in thermals. When Ignatia fails to remove the symptoms entirely Natrum muriaticum frequently comes in to complete the cure.

Nux Vomica: According to Clarke this resembles in the material desires of IGN. Both are nearby when comes to office work. But NV is bit hard and rough with others. IGN is still mild on that part.

Hyoscyamus for hysteria is called for when the mental condition of the patient exhibits marked jealousy, which is less in IGN.

Cocculus has a mood somewhat like that of Ignatia. Sensitive, anxious, with frightened look; loss of memory, mental confusion, vertigo; spasms of the uterus; great weakness and nausea even to fainting. These symptoms in Cocculus are usually the result of night watching or loss of sleep.

Pulsatilla is at times indicated for this menstrual colic, particularly when the menses are dark in color and delayed. The flow is usually fitful. The patient is apt to be chilly, and the more severe the pains are, the chillier does the patient become. The Pulsatilla woman is tearful, sad, and melancholy like Ignatia, but there is not that introspective mood which develops in the Ignatia patient.

Nux moschata is indicated in hysteria, associated with frequent emotional changes and enormous bloating of the abdomen after a light meal.

Cherianthus Cheiri - for wisdom tooth

Natural order -Cruciferae.
Cherianthus Cheiri
Common name - Wall flower.

Preparation - The fresh plant is pounded to a pulp and macerated in two parts by weight of alcohol.

(Dr. Robert T. Cooper, of London, contributed the following to the Hahnemannian Monthly, 1897.)
A tincture is used made from a single dark-flowered plant.

History

 Our knowledge of the medicinal use of Cheir. is due to Dr. Cooper. Cooper considers as a keynote of the remedy, "Nose stopped at night from irritation of cutting wisdom teeth." Planets: Mercury.

 Homeopathic

 The cases cured with it are as follows:
 (1) Young man, deafness and otorrhea left side. Is cutting three wisdom teeth. Cured by two doses of the tincture given at six weeks interValer.
 (2) Girl, 22, deaf after measles, cutting wisdom teeth. Greatly improved.
 (3) Girl, 20, used to taking purgative pills, had pain in lower back, jaundiced appearance, bilious feeling, sickness of stomach. Cheir. ch. removed the symptoms at once.

1.       Mental generals.
Mental generals were always a worry to me during college days. But thanks to the “WHY” which made many things easy. Mental symptoms are not mere collection of emotional symptoms but also the observation of the expressions of patients.

Hahnemann’s “sick man” described in detailed by Dr Kent need no introduction. We all know it is the vital energy or the living force inside every living being which gets deranged first to start for a disease. Thus the mental generals are of top importance. But only mental symptoms will not erect the totality as Hahnemann told in apho 6. A complete set of physical, mental, disease state particular and if possible PQRS will lead to a perfect similimum.
In years of practice I have learned that it is we who are wrong and not the technique. We are lacking in understanding thus gets failures.

A.      Innate characters:
WILL - emotional desires and aversion (Love, hates)
Definition: The power of choosing; the faculty or endowment of the soul by which it is capable of choosing; the faculty or power of the mind by which we decide to do or not to do; the power or faculty of preferring or selecting one of two or more objects. Every person has his own likings which reflect his uniqueness.

EMOTIONS -
Anxiety, sadness, grief, loathing of life, jealous, suspicious, depressions, weeping, irritability etc.

UNDERSTANDING -
To know thoroughly (Mental confusion, delirium, hallucination, delirium)

INTELLECT  -
Thinking power of mind (concentration, dullness; calculations)

MEMORY -
The power to remember.

B.      Reaction and sensitivity:
Every person has his individual reaction to the incident happening. If that person is sensitive enough to get emotionally charged which can derange a vital energy flow become important. These are the parameters which one can try to find in patient, emotional sensitivity like grief, criticism, lack of appreciation, rudeness, dominance etc. These are the factors to which a specific constitution can be sensitive. Thus the reaction to this sensitivity becomes very important.

C.      Ailments from:
This factor is very much commonly seen where the miasm has been changed. Thus it is responsible for conversion of miasm to make the condition worse of change the seat of the disease. Ailments from can be seen in Complete repertory under mind section. A long list of ailment will be displayed. It is mostly seen that the reaction and sensitivity or ailments comes nearby while your analysis. Ones the miasm is fully bloomed then these ailments from factors are of no use in reportorial analysis. Though, we can consider it for confirmation.

UNLESS THE INNER NATURE OF THE REMEDY CORRESPONDS WITH THE
INNER NATURE OF THE DISEASE (PATIENT), THE REMEDY WILL NOT CURE
BUT SIMPLY REMOVE THE SYMPTOM FOR A WHILE

Pyrogenium: In short

PYROGENIUM
* Inflammatory conditions, sepsis with high grade fever and great restlessness.

It is a product of sepsis, as Dr. Allen describes it.

 It is prepared from beef decomposed in water.

Guiding points:
- RESTLESSNESS (Ars, Eup-per, Rhus-t).
- Loquacious, can think and talk faster than ever before. 
- Sensation as if body is scattered in whole bed. Feels as though crowded with arms and legs.
- Feels she is one person when lying on one side and another one when lying on the other side. 
Sees a man at foot of bed on closing the eyes.
- Delusion he is very wealthy.(Plat, Sulph, Verat, Nit-ac) 

Generalities:
- SEPTICEMIA.
- Chronic complaints since severe infection. Post-operative cases with severe infections.
- Horrible OFFENSIVE discharges. The stools, the sweat and the other excretions are rendered prominent by this carrion-like smell.
- SORENESS, BED FEELS TOO HARD (Arn, Bapt, Rhus-t). 
- Burning pains. Throbbing or pulsating.
- amel. Drinking very hot water.

Organ wise:
Head - Throbbing of arteries of head amel. tight band. Bursting headache with restlessness.

Mouth - Tongue red, shiny. Coated
Stomach - Vomits water when it becomes warm in the stomach (Bism, Phos).
Abdomen - Soreness, can hardly breath.
Rectum - Diarrhea: horrible offensive, painless, involuntary.
            - Constipation: complete inertia, black balls (Op).
Cough - Cough agg. motion, warm room, lying down; amel. open air, sitting up.
Chest - Tachycardia, out of proportion to the temperature. Can hear his own heart beats.
Back - Chill begins in back, between shoulder-blades. Throbbing of arteries of neck (Glon).
Female genitalia - PUERPERAL INFECTIONS. Severe infections after abortion.
              Menses horrible offensive. Fever at each menstrual period (Rhus-t).
Extremities - Soreness, aching in all limbs and bones. Soreness amel. motion (Rhus-t).
Perspiration - Profuse hot sweat which does not ameliorate.


Stalwarts on Pyrogen:


Dr. J.H.  Hunt reports of having cured many cases of varicose ulcerations with Pyrogen that refused to heal under other drugs.

Dr. Allen's famous statement - "When the best selected remedy fails to relieve or permanently improve, Pyrogen is indicated."

Differential diagnosis
One should think of Baptisia, Arsenicum, Crotalus, Bothrops, Hepar sulphur, Mercurius sol, Arnica, Carbolic acid, Anthracinum (see article), Lachesis, Staphylococcin, Streptococcin, Echinacea, Rhus tox.
